Source Location Form


Overview

The Source Location Form facilitates the collection and submission of Latitude/Longitude coordinate information for public drinking water wells and treatment plants in the State of California that cannot be collected using the regular Department of Health Services GPS methodolgy. This coordinate information must be in compliance with the established California Department of Health Services Method Specifications for this type of information. Three types, or sources, of coordinate information can be entered into the Source Location Form.

1) Coordinate information provided by private Water Systems or other State or Local agencies. This information must meet established accuracy and format criteria and be approved by Leah Walker.

Accuracy Criteria:
GPS with a mapping grade (or better) GPS unit with differential correction.

Format Criteria:
Latitude/Longitude values in Decimal Degrees with a minimum of 6 decimal places and with a Horizontal Datum of NAD83 or WGS84

This type of coordinate informaton is entered into the "Source Location" module of the Source Location Form


2) Coordinate information calculated using the DWSAP Mapping Tool by DHS or LPA staff. Mapping Tool coordinates can only be submitted for those source or plant records that cannot be GPS'ed.

3) Coordinate information determined by identifying "virtual" treatment plant records. These are wellhead chlorinators or any treatment facility located within 10 feet of a source that has already been GPS'ed. The user will match the "virtual" treament plant record with the appropriate source record.

This type of coordinate information is entered into the "Proximity" module of the Source Location Form
