Introduction

The Source Location Form facilitates the collection and submission of Latitude/Longitude coordinate information for public drinking water wells in the State of California that cannot be collected using the regular Department of Health Services GPS methodolgy. This coordinate information must be in compliance with the established California Department of Health Services Method Specifications for this type of information (See Overview of the Source Location Form).
