The Source Location Form is not a standard Desktop application that requires an installation procedure on your computer to run. In other words, you will not need Administrator privileges. The program is simply a Microsoft Access Form inside of a Microsoft Access database.
- Requirements: Microsoft Access; WINZIP (Download from Internet); Approximately 5 MB of space.
If needed, get the assistance of your IT personnel
- Create the "SLF" directory on your computer. This will be the root directory for the application.
a. Open your Internet browser and go to this location:
- Download the Source Location Form.
ftp://swap.ice.ucdavis.edu/outgoing/b. Locate the file named slf.zip
c. Download this file by following these procedures:
- Right click on the file and select "Copy to folder" or "Save Target As" or similar.
- Save the file to the "SLF" directory created in step 2
a. Double-click the file slf.zip on your computer. The WINZIP program will appear. Hit "I Agree."
- Install the new version:
b. Hit the "Extract" button
c. In the "Folders/drives" Window, navigate to the "SLF" directory. Hit the "Extract" button.
Two Microsoft Access databases, Source Location Form.mdb and Source Location Form_Data.mdb, will now appear in the "SLF" directory
d. Close the WINZIP Application.
e. Do not ever OPEN, RENAME, or MOVE Source Location Form.mdb or Source Location Form_Data.mdb.
f. Right-Click on the Source Location Form.mdb file and choose "Create Shortcut". Drag the shortcut to the desktop of your computer.
g. Double-click the Desktop shorcut to open the Source Location Form. If the application opens with no messages then you are ready.
If the following message appears:

Then Hit "OK" and navigate to and select Source Location Form_Data.mdb. Hit "Open"

- The Organization and DHS District screen will now appear and the application is ready for use.
